Yesterday at around 1 pm there were reports that a small boat capsized near Taieri Mouth in Dunedin.
Five people were rescued and taken back to shore by local jet skiers and surfers. They applied first aid until emergency services arrived.
The outboard motor on their boat flipped as it was attempting to head out and across the bar.
One adult involved was in serious condition and airlifted with two children while a second with one child had sustained moderate injuries and was transported via ambulance.
They were taken to Dunedin Hospital. One child sadly died while another remains in serious condition in an intensive care unit.
Three people, two adults and a child were, however, released.
Police will investigate the circumstances of the death on behalf of the Coroner.
