Multiple emergency services responded to a crash on State Highway 1 between Addington and Old Hautere Roads South of Ōtaki, Sunday evening.
The crash occurred just after 4.45pm. It is believed that a vehicle had driven off the southbound lane and crashed over the bank of the railway lines across from Penrays onto the new Peka Peka to Ōtaki road before coming to a stop.
Two people were reportedly injured, including a third person who'd later died at the scene following medical assistance that was provided.

Two Fire and Emergency units can be seen parked on the slope of grass next to the railway lines where the crash initially happened.
Other emergency units such as Police and St John/Wellington Free Ambulances were in attendance.
Trains were initially stopped in the area as the scene unfolded.

Southbound traffic was backed up to Ōtaki, slowly crawling with significant delays while northbound traffic experienced some delay.
The Serious Crash Unit was called to the incident to investigate the circumstances of the crash.
